As Director of Social Services and Admissions, you are a vital member of the Hunter Acres Caring Center family and management team. Responsibilities within this position include planning, organization, and direction the overall operation of the Social Services department to ensure all medically related emotional and social needs of patients are met in accordance with all applicable current federal, state, and local standards, guidelines, and regulations, our established policies, and procedures. This role includes receiving facility referrals, securing that referral’s needs can be met at the facility, developing relationships with referral sources and implementation of facility marketing plan. Resident care plan development and meetings with resident and representatives are a core part of your career at Hunter Acres Caring Center.
The Director of Social Services and Admissions will work closely with facility Administrator and Director of Nursing. Your strong leadership will be called upon to lead the facility to success and quality of care for all residents. The ideal candidate for this position will hold a current LPN license. Prior long term care experience is preferred.
Requirements:
- Current LPN licensure in good standing
- Long-term care experience in geriatrics, behavioral health, and special needs populations
Essential Functions:
- Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines regulating long-term care facilities
- Maintains professional working relationships with community
- Maintains confidentiality of all confidential information
- Displays integrity and professionalism
- Act as patient advocate and provide education to staff regarding patient rights
- Chart appropriately and timely
- Assist patient and family through education, financial planning assistance, liaison with community agencies, etc.
- Direct social services staff
- Exhibit excellent customer service and a positive attitude towards residents and community
- Other duties as assigned by Administrator
Certified Nursing Aide

The primary purpose of the CNA position is to provide direct care to assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. Job duties must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times.
